DISCHARGING A PATIENT
Discharging plan starts after the patient is admitted to the hospital.Health education is one of the most important aspects of discharge planning.Before discharging a patient home or to other settings,it's crucial to assess patient's self-care ability and to carry out the health education.

N:Hello,Mr.Little.I am glad to know that you will go home in a couple of days.In order to help you prepare for this,I need to know more about your life at home if you don't mind.
P:Of course I don't mind.
N:Ok.I have a discharge checklist in hand.I want to make sure you will be looked after by someone at home as needed.I will also contact the GP (general practitioner)to let him/her know what happened to you during your hospital stay.
P:Ok,that is very kind of you.
N:I have your address here,is it a flat with easy access,I mean that the wheelchair can move through easily?
P:Yes,I have used my wheelchair for years.The stairs got sorted out already.
N:Great!So only the Zimmer frame is new to you?I will ask the social worker to complete a safety evaluation in your flat before your discharge date so that any potential risk will be removed.
P:That is very considerate,I appreciate it.
N:My pleasure.You will continue to take some medications and vitamins after discharge.I shall arrange for the district nurse to check with you each week about your medication,to make sure that you take the right medication at the right time and in the right amount.
P:Yeah,I think that will be very helpful.
N:Is there anything else you think you may need help with while you are at home by yourself?
P:Hmm… I could manage cooking for myself.But… but I may need some help with shopping,as I cannot lift heavy items,such as milk and juice.
N:Ok.How often do you go to the grocery?
P:Once a week.
N:Sure.I will arrange social workers to do that for you,your job is to give a shopping list to them.
P:No problem.
N:Your follow-up appointment with Dr.White has been made for 10 a.m.Wednesday,November 11th at the out-patient department,2nd floor Jeffery Wing,St.Mary's hospital.If you cannot make the appointment,please call the number on the bottom of the appointment card and reschedule.
P:I certainly will.
N:Thank you.I am going to prepare some documents for you and your GP.Meanwhile,if you need any other assistance,please do not hesitate to let me know.
P:I will,thanks a lot.
N:My pleasure.
